Job Summary:

Provide home nursing services to clients and support their caregivers. Collaborate with internal and external key stakeholders to facilitate more effective care of clients so as to allow them to age in place of their choice.

Principal Responsibilities and Duties:

  • Conduct home visits for assigned clients to assess and develop nursing intervention and care plan for clients.
  • Perform nursing procedures in home environment with resources that are available and implement where necessary with safety.

Contract Enrichment Coordinator (Integrated Family Group - TOUCH Young Arrows)

Job Summary:

TOUCH Young Arrows (TYA) is a non-profit programme led by a committed team of volunteer mentors who conduct weekly Saturday programmes to meet children and youth's social, emotional, and educational needs, and to bring out the best of their potential. There are 25 TYA clubs in Singapore serving in total 500 underprivileged children and youth aged 5-14 years old from low-income and single-parent families.

You will be part of the Learning and Development team to make TYA an innovative and sustainable model of improving the social mobility of young persons in Singapore. You will play a vital role in helping children from disadvantaged families gain access to enrichment opportunities during the school holidays and experiences that activate and develop their potential, give them a sense of self-worth, and elevate their aspirations.

Principal Responsibilities and Duties:

Learning & Development

  • Assess and select suitable, age-targeted quality enrichment camp or workshop providers in the areas of STEM/STEAM, Arts and Sports
  • Implement enrichment programmes during June and December holidays

Job Summary

Executive - Service Support (Enabling Services Hub - Jurong)

Job Summary:
The incumbent is part of TOUCH Special Needs Group who report to the Head, ESH and responsible for managing administrative tasks, assisting in the smooth operation of the centre, supporting staff and clients when necessary, and handling the maintenance of the center's facilities.

Principle Duties & Responsibilities:
1. Data Management

- Support Social Worker / Programme Executive in the preparation of relevant documents and logistics for PwDs and/or caregivers.

- Update PwDs records and prepare relevant paperwork.

- Assist in submission of documents to external agencies (e.g. SG Enable, MSF etc) and tracking of timeline for reporting.

2. Operation

- Update the various SOPs for systemic procedures such as claims and PwDs registration.

- Manage phoneline, face-to face, email and mail correspondences.

- Preparation of notes and minutes for department or any other relevant meetings.

- Manage operations of the centre to ensure smooth running which include the continuous maintenance of office premises, office equipment, office supplies and improving facilities wherever necessary.

Job Summary:

Reporting to the Head of TCW, you engage & educate youths to develop their digital literacy competencies in an ever-evolving digital landscape.

You develop relevant curriculum, content and programmes, delivered in various settings such as school classrooms and social media. Working across teams, you develop platforms, activities and new ways of engaging youths and external stakeholders to advance the cyber wellness and digital literacy agenda in Singapore. Roles & Responsibilities

Job Summary: KidSTART Lead Practitioners (LPs) play a crucial role in enabling low-income families by (1) working directly with families to address the immediate and long-term issues they face, (2) working with the surrounding community to better support the underprivileged, and (3) supervise KidSTART Practitioners (KSPs) involved in case work.

They also provide child development strategies through home visits, support groups, and ongoing monitoring. They foster parent-child bonding and promote child development by actively engaging families and working with social/ community partners and government agencies to ensure the needs of children and their families are addressed holistically.

Main Responsibilities:

  • Conduct intake and risk assessments for cases.
  • Provide case management service to clients from low-income families and/or disadvantaged backgrounds.

Job Summary:

As a Lifeskills Coach, the incumbent attend to all matters of the clients in their Form Class, at TOUCH Centre for Independent Living (TCIL), and conduct training in Activities of Daily Living (ADL) and Community Living Skills (CLS), as well as social and recreational activities to engage the clients meaningfully.

Principal Responsibilities and Duties:

A. Lifeskills Coach in Charge of a Form Class

  1. Plan and draft the Individual Care Plan of clients in Form Class based on Centre’s admission assessment, MSF Client Assessment Form (CAF), caregivers’ feedback and needs observed by other staff.
  2. In charge of administrative duties such attendance, collection of forms and fees etc.
  3. Inform the Administration Staff and Social Worker if there are changes in clients’ personal information, financial situation or records.

Job Summary:
The incumbent is part of Special Needs Group who reports to the Centre Manager of TUH and is responsible for the role of Lifeskills Coach that encompasses implementing holistic services and programmes that will benefit and make an impact to the needs of clients under TUH and Special Needs Group. The incumbent is responsible for delivery of quality service, personal efficiency and teamwork at the workplace.

Principle Duties & Responsibilities:
A. Planning, Development and Implementation of Training and Open Employment Programmes

  1. Be a part of the team for planning and development of training curriculum.
  2. Assess and chart out Individual Care Plan with social worker for each trainee under your charge at the beginning of each year and to report on progress on trainees to Centre Manager on quarterly basis and when required.
  3. Deliver lesson plans, mentor and nurture trainees in the sub-competency skills of DLS, PSS and OGP, employing creative and innovative efforts to stimulate trainees’ interests and to enhance their knowledge and skills (e.g. preparation of visual aids and materials).

  • Provide care coordination and service linkage to clients with or who are at risk of mental health and/or dementia, with the appropriate resources and support including caregiving support.
