1) What is Advance Care Planning (ACP)?

ACP is not a legal document. But it is never too late to document your wishes, personal beliefs and values early with the help of an ACP facilitator and make known your healthcare preferences in different medical scenarios.
If you are above 21 years old, you can make an ACP regardless of your health condition and change it at any time.

2) What are some reasons for making an ACP?

 

  • Gives you a sense of dignity.
  • Allows you to express your autonomy and choices to decide in advance how you wish to be taken care of before you die.
  • Spare loved ones from the burden and stress of decided for you as they can refer to it.
  • Helpful for healthcare professionals and loved ones if they know your preferences, beliefs and values.

3) Simple steps to make an ACP

1. Book an appointment by calling 9653 1076 or 6317 9996 during office hours (Mon to Fri, 9am to 6pm), or email enquirypdd@touch.org.sg if you need more information.

2. ACP Session (60 mins): Tele-ACP OR one-to-one sessions at TOUCH centres located at Bukit Merah and Ang Mo Kio

3. Completion and uploading of ACP documentation with the National Healthcare Record office by ACP Facilitator
